Who this notice applies to
This notice applies to SB DME Solutions when acting as a supplier of durable medical equipment ("DME") to you under federal HIPAA rules. It applies to protected health information ("PHI") we create, receive, or maintain about you.
How we may use and disclose your PHI
- Treatment: coordinating with your physician and other providers to deliver appropriate equipment.
- Payment: verifying benefits and billing Medicare, Medicaid, or your commercial insurer.
- Healthcare operations: quality assurance, training, audits, accreditation.
- As required by law: subpoenas, public-health reporting, abuse/neglect reporting.
- With your written authorization for any other use — you may revoke this authorization at any time in writing.
Marketing and advertising
We use de-identified or hashed data for online advertising measurement (see our Privacy Policy). We do not sell PHI, and we do not use PHI for marketing without your written authorization.
Your rights
- Inspect and copy your PHI.
- Request a correction to your PHI.
- Request restrictions on certain uses or disclosures.
- Request confidential communications (e.g. to a different phone or address).
- Receive an accounting of disclosures.
- Receive a paper copy of this notice.
- File a complaint without retaliation.
Our duties
We are required by law to maintain the privacy of PHI, provide you this notice, and follow its terms. We may change this notice; the revised notice will apply to all PHI we maintain.

You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/hipaa/filing-a-complaint. We will not retaliate against you for filing a complaint.
